    • Please carefully read the 2019/2020 Registration Information sheet (click here) and select the appropriate group in the Athlete Groups section of the registration.
    • You will be creating a log in as you go through the registration process. You do not need to create a log in and then sign your swimmer(s) up. Once you have completed the registration process, you will be sent an email to create your log in. If you listed two email addresses, you will need to create the log in for both (Please note...even though you have your own login, you will still see all of your family information).
    •  It is helpful to have email addresses for both parents if you have them. With this new program, when you sign up for an event or volunteer job, with one click of a button, we can send emails to only those people who signed up for that job. Quick...efficient...informed!
    • For the primary phone number beside each parent's email address, please use your cell phone number.
    • For the primary phone number under your home address, please use your home phone number.
    • Please have your child(ren)'s Alberta Health Care Number ready for entry on the form.

    Volunteer opt out fees are $300.00/swimmer, short course, $450.00/swimmer, long course (dated  Apr. 30/2020, or June 25/2020 if applicable).

    If your child is swimming Sept-Apr, please provide  post dated bond cheques dated for April 30, 2020. If your child is swimming Sept-June, please provide  post dated cheques dated for June 25, 2020. Please make bond cheques payable to the Olds Rapids Swim Club for $300.00/swimmer for short course, $450.00/swimmer for both short and long course.

    Olds Rapids Swim Club members and their families agree to hold the Club, it's Executive, and the coaches harmless and free from liability in event that a club member is injured or suffers serious harm as the result of a club related activity, including practices and swim meets. The sole exception being in the event of gross negligence of the Club, it's Executive, or coaches.

    Swimmer/Parent Code of Conduct

    This policy is inclusive of all levels of swimmers and their parents. Swimmers/parents should adhere at all times to standards of appropriate behavior that reflect well on the Olds Rapids Swim Club in particular, and competitive swimming in general. The Swim Alberta Code of conduct is available on the website at all times and will also be emailed to you in your registration package (please review with each swimmer/parent).

    I consent to the release of my swimmer's name, results, photos and video which may be published in the media, on the team website, Facebook, and/or on our Club bulletin board.

    Informed Consent and assumptionof risk Agreement

    Warning: By signing this document you will waive certain legal rights, including the right to sue in circumstances outlined in this agreement. Please read carefully.

    1. This is a binding legal agreement; therefore clarify any questions or concerns before signing. As a Participant participating in the sport of competitive swimming, including training competitions and practices (collectively the Activities), the undersigned, being the Participant and the Parent/Guardian of the participant (collectively the Parties) acknowledge and agree to the following terms:

The Organizer and its trainers, instructors, agents, and representatives (collectively the Organization) are not responsible for any injury, personal injury, damage, property damage, expense, loss of income or loss of any kind suffered by a Participant during, or as a result of, the Activities, caused by the risks, dangers and hazards associated with the Activities.

    Description of Risks

    3. The Participant is participating voluntarily in the Activities. In consideration of that participation, the Parties hereby acknowledge that they are aware of the risks, dangers and hazards and may be exposed to such risks, dangers and hazards. The risks and hazards include, but are not limited to, injuries from:

    a) The hazards particular to the Activities in which I am participating;

    b) Executing strenuous and demanding physical techniques;

    c) Vigorous physical exertion, strenuous cardiovascular workouts and rapidmovements;

    d) Exerting and stretching various muscle groups;

    e) Failure to properly use any piece of equipment or from mechanical failure of any piece of equipment;

    f) Contact, colliding, falling or being struck by other participants or equipment; and

    g) Spinal cord injuries which may render the Participant permanently paralyzed.

    4. Furthermore, the parties are aware:

    a) That the injuries sustained can be severe;

    b) That the Participant may experience anxiety while challenging himself or herself during the activities, events and programs;

    c) That the Participant may come into close contact with other participants;

    d) That the Participant risk of injury is reduced if the Participant follows all rules established for participation; and

    e) That the Participants risk of injury increases as the Participant becomes fatigued.

    Release of Liability

    5. In consideration of the Organization allowing the Participant to participate, the parties agree:

    a) That the Participant's physical condition has been verified by a medical doctor to participate;

    b) To freely accept and assume all such risks, dangers and hazards and possibility of personal injury, death, property damage, expense and related loss, including loss of income, resulting from the Activities;

    c) To forever release the Organization from any and all liability for any and all claims, demands, actions and costs that might arise out of the Participant's participation in the Activities, or from any breach of contract.


    6. The Parties acknowledge that they have read this agreement and understand it, that they have executed this agreement voluntarily, and that this agreement is to be binding upon themselves, their heirs, executors, administrators and representatives.
